Form and log fixes

Fixes:

1. Let form validation be handled by back-end (without front-end validation) and show message errors if exist
2. Trim leading spaces for each form field value
3. Pass correct district and sub-district value to back-end
4. Change the way to fetch investigation case logs and the pagination on Home page
