Revise choose plan logic and integrate with checkout API

Merged Wulan Mantiri requested to merge PBI-3-revision_and_integrate_with_checkout_api into staging
  • Program and nutritionist in Choose Plan are now required.

When form is filled, user is allowed to pay:

  • Nutritionists now display specialties instead of age groups.

23_chooseplan_nutritionist

  • Integrate with checkout API.
  • Use TouchableHighlight as wrapper component for Button to avoid ripple.
  • Improve coverage to 100%.
